Private Advocacy
At any given point in time, we may experience certain unexpected challenges
where issues completely overtake our lives. Those challenges could personally
impact yourself or loved ones and the combination of uncertainty is difficult
to navigate. Some concerns include quality of life, finances, medical care,
mental health, housing availability, employment and education concerns,
which can be overwhelming to anyone at any point in their adult lives.
Adding to those challenges are navigating challenging documentation
deadlines while being impacted with frightening facts and/or medical
diagnoses that can be very confusing to find your pathway through.
We at N.W. Simplelife Pathway PLLC want our clients to know that
their voices and thoughts are heard and that their views matter, greatly.

and does lead toa sense of isolation. We understand it can be not easy
to speak up which leads to not feeling heard and not getting the help you need.

(COMMUNITY CHOICE GUIDE)
What is a Community Choice Guide?
Community Choice Guides (CCG) are Independent Living (IL) essential
providers who assist clientele during transitions from institutional skilled
nursing settings with the approval of the client's DSHS casemanger.
CCGs also provide assistance in the client's residential placement
choices by working the the client to appropriately, identify, independent,
living settings.
This may also include connecting client's with caregiving services
should care assistance be needed upon discharge.
Firstly, the CCG will provide remote or direct consultation interviews
with the client to determine if clients are interested in moving out
of skilled nursing facilities, such as hospitals and nursing facilities,
into Assisted Living Facilities, into Adult Family Homes and
affordable apartment complexes.
Secondly, CCG(s) provide client pre-transitional consultation
services, assess for client interest in transitioning from
institutionalized care settings.
Tours of residential placements may be conducted through
video recording, live video phone call, through use of photographs, or
directly with the person based on the client's ambulatory abilities.
These services may include but are not limited to:
1. Locate Appropriate Housing.
2. Assist client in filling out applications.
3. Paying appropriate application fees, and or utility connection
fee (may include assisting with first-last-security deposits
based upon Casemanager approval),
4. Coordinate move in dates with movers and client
5. Create a list with client to include transitional housing needs.
(may involve, furniture, cooking utensils, groceries,
bathroom items and bedding item).
6. Deliver and set up new living space with furniture and
household items.
CCG responsibilities may include but are not limited to the following:
o Creation of an individualized transitional placement plan.
o Assist clients during discharge and transitioning into
community housing placement.
o Assist in stabilizing client's housing placement by providing
consistent support and assistance to protect client's safety,
welfare and physical and psychological needs.
o Assist in locating transportation services to assist client's daily life needs,
medical care needs, pharmacy needs and mental health needs.
This may involve connecting client's with Intercity Transit Bus Passes,
Dial - a - Ride and Paratransit services.
o Locating and connecting clients with primary care providers
or long term mental health providers.
o Assist client in completion of appropriate housing placement
applications, vouchers and when relevant, act as a liaison with
and among the client, nursing or institutional facility staff,
case managers, housing providers, medical personnel, legal
representatives, formal caregivers, family members,
informal supports and any other involved party.
o Research possible non-traditional supports to assist client
in adjusting to the new community placement, such as
connecting with local senior centers and other supportive
services.
o Assist clients in identification card replacement, social security
card replacement, and assist client in arranging utility
hook-ups or acquisition of furnishings and household goods.
